Students with disabilities want to become self sufficient, productive members of society. During the transition from high school, many are learning to be independent, preparing for college, or joining the workforce. IDEA requires schools to include a transition plan in IEPs for all students age 16 and older. The plan outlines the supports and transition services to help students reach their goals for living, working, and participating in the community after high school.
